A Power Of Attorney For Health Care Form allows a person (the Principal) to designate another person to make health-care decisions for them if they become incapable of making their own decisions. The designated person is called the attorney-in-fact or agent and generally they tend to be family members or friends, although anyone else can be chosen. The powers given to the agent are defined in the power of attorney form and can be fairly broad or limited. A health care power of attorney can be revoked at any time by a competent principal.
