This Corporate Function Release is between a participant and a company who is sponsoring an event or function. This document effectively releases the corporate sponsor or company from any liability due to an accident caused by the over consumption of alcohol at the function. It also holds the sponsor harmless from any claims connected to attendee's participation at the event. Each participant must sign this release in front of a witness prior to attending the event. A well-written Corporate Function Release will prove invaluable in the event a participant at an event suffers death or injury due to an accident and litigation is filed.

This Corporate Function Release contains the following:

• Sponsor: Name of the company or corporation sponsoring the event;
• Function: Type of function (i.e., holiday party, farewell dinner) and the date the event will be held;
• Releases: Participant agrees not to consume excess alcohol at the function and if consumed, agrees not to drink and drive. Participant also releases sponsor from any claims with respect to death, injury, loss or damage arising from participation at the function;
• Signatures: Participant must sign and date this release in the presence of a witness and by signing, acknowledges and agrees with the terms of the release.
